On behalf of the American Association of Kidney Patients (AAKP), we are pleased to invite you to participate in our online Survey for Patient Education of Renal Replacement Therapy Options. The purpose of this survey is to gather valuable feedback from the viewpoint of patients and will explore what type of pre-dialysis education patients are receiving and how they are receiving that information.

AAKP realizes that End Stage Renal Disease (ESRD) patients have several treatment options to choose from: transplant, in-center hemodialysis, home hemodialysis, peritoneal dialysis, and nocturnal dialysis. Your responses will provide us with vital information about who is providing you with pre-dialysis education, how you are receiving the information and why you chose a particular treatment modality over another. Your responses to the survey questions are completely confidential and you are not required to input any personal contact information. Participation in the survey is easy and should only take a couple of minutes to complete. It is our goal to reach 100% survey response rate, so it is vital that your responses be received before Wednesday, July 2nd.
