To register, click on Register Here. Please fill in your name, mailing address with zip code and phone number. Choose a password and make sure you write it down on a separate sheet of paper. You will also be asked to select the type of patient you are (modality): chronic kidney disease, dialysis or transplant. It is very important that you select one. The charts you have access to in My Health are based on your modality. If you change modalities (start as a dialysis patient and later receive a transplant) after you've registered, remember to come back and update your profile.

During the registration you can also select to receive additional free information from AAKP. The information you provide is only used to ensure you receive the most appropriate information. AAKP will never rent, sell or trade your personal information to third parties.
